We have had this in the shop off and on since October EGR valve has been replaced twice. The throttle position sensor once. The PCM was replaced by Dodge. Mechanics have checked wiring and grounds and seems to still have a problem. Has a remote start put in a year ago but problem did not start until 6-7 months after that install. What else could be the problem? We have now replaced the map sensor as well as the TPS sensor, both battery cables removed and cleaned most grounds. The PCM has been flashed again for a second time. This is driving us and the Mechanic nuts as they cannot seem to get this problem resolved code will stay away for a day then return once we get the van back. Another issue seems that the van seems to be idleing fast like when you first start it. Also it seems to shift hard on occasion between gears. The problem is usually more common at slower speeds but has happened at highway as well a few times.
Hi Shawn, There is a multi-step procedure outlined in the service manual for troubleshooting the 0406 code. I can copy those pages and the wiring diagram for the egr valve and attach those to an email which I send directly to you if you would like to have those pages to try yourself. It would mainly require a volt-ohm meter.

Other than going through the troubleshoot yourself I can offer nothing from personal experience of that of other owners.
