Looking up the date

What I am trying to do is look up the date (from column "B")for each instance a patient has called (using the patient number, 10201,10202 etc.)also looking under another column (same row)for the name of the visit IE; Month 1, Month 2, etc.

I want it to populate a sheet I made to track these dates per patient.

If you want to get all dates for all patients, then a pivot table may be the way to go - you didnt specify version ofso it's difficult to give specific advice, but effectively this would (presumably) show patients down the left side, description of visit along the top and the date (as a max value) in the middle.
